I the parent/guardian of the minor Player, acknowledges that soccer is a physical sport in which the Player participates at his/her own risk. I hereby release and indemnify Juventus Camp and its organizer Golden Goal Sports & Marketing, their officers, directors, partners, coaches, team managers, volunteers, agents, representatives and assigns, from and against all claims, liabilities, damages or causes of action arising out of or in connection with the Player's participation in this camp.
I affirm that the Player is in good physical condition. I understand that the event does not carry medical insurance for Players participating in this event and that I am responsible for the Player's insurance coverage.
Camp fee is non-refundable, except for a medical reason if participant is sick or injured. I understand that Juventus Camp offers $25 Cancellation Protection plan for full refund (minus $8 processing fee and $25 Cancellation Protection fee) for cancellation requests received 7 or more days before the camp start date. If requesting refund for medical reason, a doctor's note must accompany cancellation request sent to the camp's email address. If approved, the organization will retain $75.00 as a compensation for the registration expenses. I also understand that the event organizer can cancel the event, and in that case full refund will be issued.
